CANINE CAMPUS (DBA Camp K9)
DAYCARE & BOARDING LIABILITY WAIVER, RELEASE & SERVICE AGREEMENT
Facility Location: Mishawaka, Indiana
Please read carefully before signing. This Agreement sets the terms under which Camp K9 “Camp K9,” “we,” “us,” or “our” provides dog daycare, boarding, enrichment, transportation, grooming, and related services.
By signing, the Owner confirms they have read, understand, and voluntarily agree to all terms below.
SERVICES
Camp K9 may provide any of the following services as requested or scheduled:
Daycare, Overnight boarding, Enrichment activities,One-on-one training or enrichment
Group play and socialization, Crate/rest periods, Grooming and nail services, Transportation, Feeding and medication administration only if agreed, Other services offered by Camp K9, Camp K9 may decide whether a dog is suitable for any service based on health, behavior, temperament, size, age, energy level, social compatibility, and safety.
INHERENT RISKS
The Owner understands that dog care and group environments involve risks that cannot be fully eliminated. These risks include, but are not limited to:
Dog fights, bites, or altercations, Scratches, sprains, strains, or other injuries, Injuries from play, running, jumping, or slipping, Illness or exposure to contagious disease, Stress, anxiety, or behavioral changes, Escape or temporary loss of the dog, Injury involving equipment, crates, fencing, toys, or doors, Allergic reactions or ingestion of foreign objects, Worsening of unknown or pre-existing conditions, Injury during transport or grooming, Medical emergencies, Death. The Owner understands these risks remain even with reasonable care and safety procedures.
ASSUMPTION OF RISK & RELEASE
To the fullest extent allowed under Indiana law, the Owner voluntarily assumes all risks related to Camp K9 services, including daycare, boarding, transport, grooming, enrichment, and socialization. The Owner releases Camp K9, its owners, employees, contractors, and agents from claims arising from these inherent risks, including injury, illness, loss, or death of the Dog, except where such release is not legally permitted.
This release applies to ordinary risks associated with pet care services.
DOG-TO-DOG INTERACTIONS
The Owner understands that dogs will interact with other dogs during services.
Even well-behaved dogs may react unpredictably.
Camp K9 will use reasonable judgment in managing group play but cannot guarantee that incidents will not occur. Camp K9 may separate, crate, redirect, or remove a dog from group play at any time for safety reasons.
BEHAVIOR DISCLOSURE
The Owner agrees to fully disclose all known behavioral or safety concerns, including:
Aggression or reactivity, Bite history, Resource guarding, Fear, anxiety, or separation issues, Escape behavior, Handling sensitivity, Food guarding, Dog or human reactivity
Past incidents involving people or animals, Medical or behavioral conditions, Medication needs, Any behavior affecting safety. Failure to disclose may result in termination of services and may impact safe care.
RIGHT TO MODIFY OR END SERVICES
Camp K9 may modify, restrict, suspend, or end services at any time if a dog’s behavior, health, stress level, or other conditions create safety concerns.
This may include:
Removing the dog from group play, Increasing crate/rest time, Providing one-on-one care only, Requiring evaluation or veterinary clearance, Requiring immediate pickup, Refusing future services. Camp K9’s safety judgment is final.
HEALTH & VACCINATIONS
The Owner confirms the Dog is healthy and current on required vaccinations.
The Owner must provide accurate health records and update Camp K9 of any changes.
Dogs showing signs of illness may be refused or separated, including:
Coughing, Vomiting, Diarrhea, Fever, Lethargy, Respiratory illness, Contagious skin, conditions, Other symptoms posing risk, Veterinary clearance may be required before return.
ILLNESS EXPOSURE
The Owner understands that exposure to illness is possible in group care settings even with sanitation and vaccination protocols.
The Owner accepts this risk and agrees Camp K9 is not responsible for illness resulting from ordinary exposure, except where prohibited by law.
VETERINARY CARE & EMERGENCIES
If Camp K9 believes veterinary care is needed, it may seek treatment for the Dog.
In emergencies, Camp K9 may transport the Dog to a veterinary facility.
Camp K9 may make reasonable emergency decisions if the Owner cannot be reached.
The Owner is responsible for all veterinary and related costs.
Camp K9 staff are not veterinarians and do not provide medical diagnosis or treatment beyond basic care and emergency response.
PRE-EXISTING CONDITIONS
The Owner must disclose all known medical, behavioral, dietary, and allergy-related conditions before services begin.
Camp K9 is not responsible for complications or worsening of undisclosed or pre-existing conditions, except where required by law.
MEDICATIONS
Camp K9 will only administer medication if it agrees to do so.
Medication must be provided in original packaging with clear instructions.
The Owner is responsible for correct dosage and instructions.
Camp K9 does not guarantee a dog will take medication without difficulty.
FOOD & DIET
The Owner must disclose all dietary restrictions, allergies, and feeding instructions.
Camp K9 will make reasonable efforts to follow instructions but may use treats or enrichment items unless specifically prohibited in writing.
The Owner must report all food-related risks or allergies.
ESCAPE OR LOSS
Camp K9 uses reasonable precautions to prevent escape.
However, no facility can guarantee a dog will not escape.
If escape occurs, Camp K9 will attempt recovery and notify the Owner or emergency contact.
PROPERTY DAMAGE
The Owner is responsible for damage caused by the Dog to Camp K9 property, equipment, or facilities, as allowed by law.
This includes damage from chewing, digging, scratching, or destructive behavior.
PICKUP & ABANDONMENT
The Owner must pick up the Dog on time and remain current on payments.
If the Dog is not picked up or the Owner cannot be reached, Camp K9 may take reasonable legal steps, including contacting emergency contacts or authorities.
The Owner remains responsible for all boarding and related costs.
PAYMENT
The Owner agrees to pay all service fees, including daycare, boarding, grooming, transport, medication, veterinary, late fees, and cancellations.
Unpaid accounts may result in refusal of future services.
PERSONAL ITEMS
Camp K9 will use reasonable care with personal belongings but is not responsible for loss or damage unless required by law.
PHOTOS & VIDEO
Camp K9 may take photos or videos of dogs during services.
Camp K9 may use these for marketing, social media, and promotional purposes.
OWNER AUTHORIZATION
The Owner confirms:
They are legally authorized to sign.
All information provided is accurate.
All known risks, medical, and behavioral issues have been disclosed.
They understand inherent risks of dog care services.
They agree to follow Camp K9 policies.
Camp K9 may modify or end services for safety reasons.
INDEMNIFICATION
The Owner agrees to cover and hold harmless Camp K9 from claims, losses, or expenses caused by failure to disclose information about the Dog or the Dog’s behavior, except where prohibited by law.
LIMITATION OF LIABILITY
Camp K9’s liability is limited to the maximum extent allowed under Indiana law.
Nothing in this Agreement removes rights that cannot legally be waived.
UPDATES
The Owner must promptly update Camp K9 regarding:
Contact information, Emergency contacts, Veterinary details, Vaccinations, Medical or behavioral changes, Medications, Allergies, Feeding instructions, Ownership or authorization changes
GOVERNING LAW
This Agreement is governed by Indiana law.
If any part is unenforceable, the rest remains valid.
ACKNOWLEDGMENT & SIGNATURE
By signing, the Owner confirms they have read and understand this Agreement and agree to all terms.
The Owner understands dog care involves unavoidable risks.
The Owner confirms all information provided is accurate and agrees to follow Camp K9 policies.
